A GP chronic condition management plan (GPCCMP) is prepared with item 965 and reviewed with item 967, in person. By video, they’re 92029 and 92030. A prescribed medical practitioner (a non-VR doctor) uses 392 and 393 in person, and 92060 and 92061 by video. Item 965 Item 967 AN.0.47

These plans replaced GP management plans (721) and team care arrangements (723). A patient whose old plan was made before 1 July 2025 gets a new plan, not a review. AN.15.5

Phone versions start on 1 November 2026: 92033 to prepare and 92034 to review, and 92063 and 92064 for prescribed medical practitioners. Until then, the telehealth versions are video only. MBS release of 1 November 2026 Item 92029

A plan prepared or reviewed in the last 18 months keeps the patient’s practice nurse services (10997) and allied health services available. MN.12.4 AN.0.47
